All Choate Rosemary Hall students are required to have health insurance (covering both physical and mental health) prior to registration in September. There are many ways to minimize the cost of medical care for your child. Choate Rosemary Hall does not have a health fee (most schools do). We will bill you for those supplies, treatments and services that should normally be paid for by health insurance. This includes prescription medications, orthopedic supplies (such as splints), physician and nurse practitioner visits, laboratory charges, and treatments such as nebulizers or sutures. Charges unpaid by insurance companies (for any reason) and co-pays will be billed to your child's school account. We do not bill for over the counter supplies (Tylenol etc.) or overnight stays.
If we need to refer a student to either a private specialist including the orthopedist who comes to the Health Center weekly, or to a hospital, you will be responsible for any portion of the bill not covered by your insurance. As I am sure you realize, a simple appendectomy can result in charges up to $5,000. More complicated surgery can be far more costly. To cover the unlikely but very expensive possibility of a student's needing surgery, lab work, or therapy, we must ascertain that your child is covered by private insurance or by one of the school's insurance plans. We offer several plans through Hilb, Rogal, and Hobbs Company of Connecticut that may be of interest to you. These are outlined below and in the enclosed brochure.
Plan A: Partial primary insurance: This should pay for 80% of the usual and customary charges up to $1,000 for any covered illness or injury that occurs after the effective date of the coverage. This coverage is in addition to other health insurance you have for your family. Cost: $250.00
Plan B: Primary insurance: International students are required to enroll in this plan. This should pay for 80% of the usual and customary charges up to $25,000 for the treatment of any covered illness or injury that occurs after the effective date of the coverage. Effective this year, this plan includes a wellness provision up to $200 per year, including physical examination, if requested. Cost: $1,022.00
Plan C: Supplemental Benefit (overall $100,000 maximum with 20% coinsurance only available to those enrolling in Plan B above.) After a deductible of $25,000, this should pay for 80% of the usual and customary charges up to $100,000 for the treatment of any covered illness or injury that occurs after the effective date of the coverage. Cost: $ 219.00
Please review your primary insurance for:
- a. Out of Network coverage. Dr. Gardner is in the network of Aetna, Anthem Blue Cross/Blue Shield, Health Net, Connecticare, and United HealthCare. (We urge you to contact your insurance carrier to check our participation in your plan; even though we may be in the network of your insurance company, we may not be participating with your particular plan.) If you have any other insurance coverage, you may well consider the school insurance plan.
- b. Deductibles. If you have a deductible on your policy, you may well consider the school insurance plan.
- c. Primary Physician. If you are covered by one the insurance companies who has Dr. Gardner in their network, you may be able to designate him as your child's primary physician during the school year, obviating "out of network" or "deductible" exceptions.
Therefore, in order for your child to register for school in September, we must have either:
a. the insurance application form completed and returned with payment to: Hilb, Rogal and Hobbs Company of Connecticut as soon as possible but no later than September 1
OR
b. if your child is covered by private, U.S. approved health insurance, you must provide an enlarged, clear copy of both sides of the insurance card, which will serve as proof of that coverage and complete the enclosed application form declining coverage, and return it directly to the Health Center.

Even if you have your own health insurance, we still recommend Plan A above to help fill in those voids (out-of-network, deductibles, non PCP) in regular insurance. Please note: As of the date of this letter, Cigna will not pay for services at our Health Center.
