- What if I have other insurance in addition to the student plan?
- It depends on which plan is selected, you can refer to the student insurance brochure for the information.
- Am I covered on school breaks, summer vacation, studies abroad?
- Yes, the plan covers you anywhere in the world.
- Does the plan cover pre-existing conditions?
- It depends, if an insured person has been continuously covered under a health policy from year to year the pre-existing condition will be covered (as long as it is a covered benefit), but if an insured person has had a lapse in coverage of more than 63 days than the pre-existing condition will be excluded from coverage for 12 months following the effective date.
- Can I waive off the plan if I have other health insurance?
- Yes, however proof of medical insurance has to be submitted with the completed waiver form.
- Can I use my own doctor?
- Yes, however the student insurance plan does participate in the Health Choice of CT PPO network. The PPO providers agree to treat insureds for a reduced fee over the customary fees of non-PPO providers. If your doctor/hospital is not in the network you may be subject to out of pocket expenses.
- Will I get an ID card?
- Yes, the ID cards are mailed to the school.
- What if I leave Choate, can I get a refund?
- To qualify for a refund, a student must withdraw from Choate within 31 days of the period for which the coverage is purchased.

- When I graduate, will my insurance offer a Continuation?
- No, you have to be a full time student at Choate to continue to be covered under this plan.
- How can I get a copy of the brochure and enrollment form?
- If you have the Acrobat Reader on your system you can download the entire packet from this site. You may also request a packet from HRH by calling (800) 624-4184x407.
- When may I enroll?
- You may enroll during your schools open enrollment period. If you wish to enroll after the above dates you can contact HRH.
- Does the student insurance cover routine check-ups and immunizations?
- No, the plan does not cover routine or preventive visits.
- How do I file a claim?
- When you become enrolled in one of the plans, you will receive a confirmation letter along with a small supply of claim forms. Complete the claim form, attach copies of the provider bills and mail the completed information to Consolidated Health Plans, the address is at the top of the claim form. Claim forms are also available on this site.
