How to File a Claim

You must submit a claim form with each new illness or injury. This may be obtained from HRH, your school or on this site.

At the providers office you need to inform the billing person that claims will be processed by:

If the provider participates in the HMC, PPO (formerly Health Choice of CT) network then the doctors office will submit your claim for you, if the doctor is not a participation provider then you may have to pay for your visit up front (depending on your doctor) and be reimbursed by Consolidated Health Plans. For reimbursement of your claim you will need to send a completed claim form along with an itemized bill and proof of payment to Consolidated Health Plans at the address above.

Steps to Ensure Quick Payments of Claims

At Health Services

  • Student Claim Forms: You MUST submit a Student Claim Form with each new illness or injury. This may be obtained from Health Services. Your claim will not be processed until this form is received by Consolidated Health Plans (CHP).
  • HMC, PPO Provider Directory: This directory lists providers that participate in the provider network. You may call HMC, PPO at (866) 575-5553 to inquire if a specific provider participates, or you can visit their website at http://www.bsi-vt.com/hmc-ppo. An updated directory is also at Health Services for your review.

At the Providers Office

  • Inform the billing person that claims will be processed by:

  • Hilb, Rogal & Hobbs does not process claims. For benefit verification or claim status the provider's office should contact Consolidated Health Plans (CHP) at (800) 633-7867.
  • The claim from the provider's office produces is not the same as the Student Claim Form. If the providers office is going to send their claim form directly to CHP, please keep in mind you must still send the Student Claim Form.

Waiting for a Claim to be Paid

  • You may call CHP at (800) 633-7867 to check on the status of a claim.
  • If CHP is missing information, they will contact the appropriate parties for the needed information. They will request the information 3 times. If they do not receive a response for the requested information no further action will be taken on the claim.
